Step 1: Visit the official website of Philhealth

At the home page navigation menu, go to “Download.” It is located at the rightmost part of the green navigation menu next to “online services.”

The available forms are in PDF format. Download whichever suits your needs. These are the available forms:

  • Philhealth Member Registration Form (PMRF)
  • Claim Form 1 for Member and Patient Information form (CF1)
  • Claim Form 3 for Patient’s Clinical Record (CF3)
  • Philhealth Premium Payment Slip (PPPS)
  • Claim Form 4 for Clinical Summary (CF4)
  • Employer’s Data Record (ER1 to ER3)
  • Household Employer and Employment Unified Registration Form
  • Kasambahay Household Philhealth Payment Slip (PPS)
  • Claim Form 2 for Provider Information (CF2)

Step 3: Print the PhilHealth forms

These are the steps in order for you to print the forms with respect to the paper size required by the health insurance institution.

Assuming you do not have a PDF viewer, you can get an adobe acrobat reader online for free at their official website https://get.adobe.com/reader/. Install the software on your personal computer.

Open the file using the PDF reader. At the upper left side of the window, you will see the printer icon. Run it to open the printer popup window.

Select the active printer that you want to use. Afterward, proceed with settings adjustment. Check the paper orientation. It should be in portrait mode. Check the margin. Make sure that the paper size is just right based on the PhilHealth requirement.

You can print 1 page first to check the actual outcome according to your printer settings. Make adjustments if the outcome is not OK. Otherwise, print as many copies as you like. Since it is free, you can also give it to your friends and to those who needed it the most.

    I know PhilHealth was one of the best health card…I used to be a member….when I had my early retirement….my philhealth was besing stopped….I want to be a member again….was it possible if I will just fill up thru my desktop!….I am already 61 yrs old…

    Good day po..just want to ask po. na confine po kase ung son ko last dec. 25-28, 2012 ang findings po ng dr. is accute gastroenteritis mild dehydration. then nung ma ddischarge n po kmi s hospital biglang sinabi po s billing section ng hospital hndi daw po covered ng philhealth ung findings. anu po reason bkit hndi covered yun? according to my files here in our office included po cya s philhealth implements new case rate..
