PRC Authorization Letter Sample and Format

PRC registered professionals who cannot claim their ID or board certificates can assign a representative to collect them on their behalf.

The representative should provide to PRC an authorization letter or special power of attorney, a photocopy of both his/her valid ID and the ID of the professional he/she is representing. The IDs should show their respective signatures.

There are times when professionals and OFWs working abroad will not be able to collect the documents they requested from PRC. Often, because their vacation leave did not match the date they will visit PRC.

If this is the case, the professionals can send a friend, or relatives to the PRC branch to claim the certificates for them. The latter needs to bring valid IDs and an authorization letter.

Here is a sample format of an authorization letter that your representative can bring to the PRC to collect your Professional ID card or any PRC-related certificates. PRC did not specify if the authorization letter should be hand or typewritten. So, you can handwrite or type it using Microsoft word. What’s important is that you state clearly the purpose of the letter, the complete name of your representative, and your signature is also found on it.

Some professionals usually have a laptop or desktop at their place but they may not have a printer or scanner at home. As an alternative, you can handwrite the authorization letter and just take a decent photo of it using your phone. Send the photo to your representative in the Philippines so they will be the ones to print it.

There you have it guys! The people in PRC understand that registered professionals may or may not be able to claim their documents personally on the specified schedule for various reasons.

    How about for initial claiming of the PRC license for those who just took their oath? May we also send a representative to pick up our license with a special power of attorney and photocopy of government ID? Thank you so much!
