The SSS E1 form is necessary if you plan to apply for an SS number because it is part of your pre-employment requirement. On the other hand, you need the same E-1 form if you will apply for an sss number under voluntary membership such as a self-employed, unemployed spouse, or as an Overseas Filipino Worker (OFW).
The distinct difference between the two membership types is the information that you are going to input on the e1 form when you apply for social security number.
To give you an idea, if you will apply for sss membership as an employee, you have to fill out the Personal Data (Column A), Dependents and Beneficiaries (Column B), and Certification (Column D). You have to leave the “For Self-Employed / OFW /Non-working spouse” (Column C) blank.
Meanwhile, if you will apply for sss membership as a Voluntary, self-employed, OFW, or unemployed spouse, you have to fill out all columns from A to D.
The SSS Personal Records from (E1) has undergone series of revisions. From what I have observed, in the old form, there is only Part 1 and there is no Column C.
On the new version, SSS divides the form into two parts and they include Column C which is for those who will apply for SSS number as Self-Employed, OFW and Non-working spouse.
New members have the option to input their Tax Identification Number (TIN). For OFWs, they can input the telephone number that they use abroad. Moreover, sss has placed the location of the social security number on the upper right-hand side whereas it is located on the upper left-hand in the old form.

How to Get a Copy of SSS E1 Form Online
Step 1. Visit the SSS homepage and Go to Forms Link
The sss homepage is www.sss.gov.ph. Scroll to the footer of the page until you see the “Forms” link. It is located adjacent to the “Corporate Governance” link. Go to the forms link to view the different sss forms that you can get online. Note: getting an E1 form online does not require registration.
Step 2. Select the Personal Data Record E1 form in the list
On the new page, you will be able to see the sss downloadable forms. Under the “Member Forms” column. Proceed to the “Personal Data Record” link to view the E1 form.
Note: You can also view the other forms such as “Member’s Data Change Request form, Unified Multi-purpose ID (UMID) card application form, Kasambahay Unified Registration form, and Verification Form for SSS number and contributions.
Step 3. Download and Print the SSS E-1 Form
You have to option to download the form and save the pdf copy on your computer. Or you can choose to print the e-1 form immediately.
To do so, at the upper right-hand part of the browser. Hit the “Printer” icon. Then select the printer you will use, the number of copies, orientation (portrait/landscape), and the page number. When everything is set, print your e1 form. The paper size that you can use is 8.5 x 14 inches long bond paper.
The SSS e-1 form is free and is not for sale. If you lost your copy then just print as many copies as you want. Fill up the form in all capital letters with a blank ballpen. Then submit it together with other requirements to the nearby sss branch for your sss number application.
